The SEED is a not-for-profit food project at the Guelph Community Health Centre that was 
created by a coalition to work towards a community with no barriers to healthy food. The SEED delivers programs and operates a social enterprise that address the causes and effects of food security.  
 
Groceries from the SEED is a pay-what-you-can online grocery store with a purpose! When you shop at Groceries from the SEED you help people in your community afford good food. Delivery Crew Volunteers are critical to the success of this program. Delivery Crew Volunteers pick up Groceries from the SEED baskets at the packing warehouse and delivery them to community members homes. Volunteers are required to drop groceries off at the door and do not enter homes.  
 
Use of personal vehicle and cell phone are required for this role. Mileage compensation of use of personal vehicle is $6 per delivery route (6-10 stops) and paid quarterly. Self-scheduling with online spreadsheet is required.  
 
 
IMPORTANT Information: This role can be done in pairs with people from the same household OR willingness to ride along with another driver in The SEED's cargo van.

Description: Direct patients to next available Intake Station 
Location: Reception area of Skyjack Centre 
Required PPE: Level 2 medical mask and face shield, provided by clinic

Roles and Responsibilities: 
• Directs patients to next available Intake Station

Description: Direct patients to vaccination stations

• Greets patient when they enter line up and ensure everyone is spaced appropriately 
• Asks patient to confirm appointment time 
• Asks patient if they have on their person their health card or alternative ID (accepted IDs listed in COVax, a software created and provided by the provincial government for COVID-19 vaccine administration) 
• Ask the 3 COVID questions on document 
• Direct patients into vestibule when empty

Description: Check in patients using COVax and directs them to the vaccination area  
Location: Check-in stations  
Required PPE: Level 2 medical mask and face shield  
Required Materials: Laptop, monitor, mouse, laminated health care sheet, laminated consent statement, laminated list of required fields for check-in, disinfectant wipes, hand sanitizer  
Roles and Responsibilities:  
• Coordinates with check-in traffic volunteer to direct patients to their empty check-in station  
• Greets patient when they arrive to their check-in station  
• Asks patient to place their health card or alternative ID (accepted IDs listed in COVax) on designated sheet and slide it across table to prevent contact with patient  
• Remind patient to stand on the marker (if necessary) during check-in  
• Confirms that patient is by cross-referencing appointment list  
• Searches patient in COVax and confirms that all patient information is correct including:  
o First Name, Last Name  
o Health Card Number or Alternative ID  
o Date of Birth  
o Gender  
Recommended verbiage: “Please confirm your date of birth and gender. I want to confirm that I found the correct person in our system”  
o Address  
Use Google search function

• If the patient is not in the system, a new profile is created using the “Search HCN in Provincial Registry” and saved in COVax

1) Consent for electronic data collection in COVax  
 Consent statement laminated and available for patient to read  
 If patient does not consent, flag the COVax lead  
2) Record preferred method of follow-up communication (email is suggested as this communication will include their receipt of vaccination)  
3) Record preferred method of contact if patient wants to be contacted about research studies for the COVID-19 vaccine  
• Directs patient to the line for vaccination area  
• Wipe down health card sheet with disinfectant wipes between patients  
• Wipe down table and computer with disinfectant wipes when leaving for breaks, returning from breaks, and leaving after shift is completed  
• No food or drink is allowed at stations

Description: Check out patients using COVax, provide them with proof of vaccination, and direct them leave the building  
Location: Check-out stations  
Required PPE: Level 2 medical mask and face shield  
Required Materials: Laptop, printer, mouse, laminated health care sheet, disinfectant wipes, hand sanitizer  
Roles and Responsibilities:  
• Coordinates with recovery volunteer to direct patients to an empty check-out station  
o If no check-out station is available, recovery volunteer will direct patient to line or ask them to remain in recovery  
• Greets patient when they arrive to their check-out station  
• Ensures that patient has completed their designated waiting period in the recovery area  
o White sticker: 15-minute wait time  
o Red sticker: 30-minute wait time  
• Remind patient to stand on the marker (if necessary) during check-out  
• Ask patient how they are feeling  
o If patient is feeling faint/lightheaded/unwell, coordinate with recovery volunteer to find an EMS first responder from recovery area  
o DO NOT let patient leave building if they are feeling unwell  
• Searches for patient in COVax and ask patient to confirm date of birth  
o Client status should be at “Dose 1 (or Dose 2) Administered”, if not flag COVax lead  
• Open client profile and confirm their email address (ensure spelling is correct)  
• Clicks the “Check-out” button on COVax and completes check-out procedure  
1) If an adverse event occurred (assume no adverse event unless patient or nurse notifies you)  
2) If box for email receipt is available, always check this box  
• Ask the patient if they would like a physical receipt of their proof of vaccination  
o Download the PDF file from the “Files” section of the patient’s profile  
o Print the receipt and allow the patient to remove the file from the printer  
• Direct the patient to the exit  
• Wipe down table and computer with disinfectant wipes when leaving for breaks, returning from breaks, and leaving after shift is completed  
• Make sure that no proof of vaccination PDF files are saved to the laptop at the end of your shift – delete these files if they have been saved  
• No food or drink is allowed at stations

Description: Assist clinic patients with direction and ensuring they are only entering the queue for their appointment times  
Location: Parking areas and Entry Line  
Required PPE: Level 2 medical mask  
Required Materials: Orange Safety Vest and Level 2 Medical Mask  
Roles and Responsibilities:  
• Ensure empty parking spots can be identified to patients arriving at clinic  
• Ensure patients understand where to line up for entry to the clinic  
• Ensure the entry line is in order of appointment times  
o Patients if left to their own will line up first come first serve  
o We need to ensure they are in order by their appointments  
o The person whose appointment is at 10:30 should be in front of the 10:45 patient  
o This will ensure better flow inside of the clinic  
• Periodically monitor patients in line to assist if required – Do not touch patients physically – get them a chair if necessary  
• Provide direction back to parking lot, if necessary  
• Ensure volunteers are parking at Hitachi and not in clinic parking lots  
• In case of Emergency  
o If you are first on scene notify supervisory personnel  
o Advise all people - emergency in process  
o Ensure any patients outdoors are aware there is an issue and advise of possible wait time  
o If requested, assist incident commander or emergency team as directed

Location: Vaccination stations line  
Required PPE: Level 2 medical mask and face shield, provided by clinic  
Required Materials: disinfectant wipes, provided by clinic 
Roles and Responsibilities:  
• Coordinates with check-out volunteers to direct patients to an empty check-out station  
o If no check-out station is available, recovery volunteer will ask them to remain in the recovery area  
• Ensures that patient has completed their designated waiting period in the recovery area  
o White sticker: 13 minute wait time  
o Red sticker: 30 minute wait time  
• Remind patients not to stay longer than their designated waiting period  
• Observe how patients feeling  
o If any of the patients in the recovery area is feeling faint/lightheaded/unwell, coordinate with recovery volunteer to find an EMS first responder from recovery area  
o DO NOT let patient leave recovery if they are feeling unwell  
• Direct the patient to the check-out stations  
• Wipe down recovery chairs with disinfectant wipes between patients  
• Ensure 1.5 minutes gap between disinfecting chair and next patient  
• No food or drink is allowed at stations and in the line

Volunteers are making it a special day by writing positive messages that are being sent to clients or included in Groceries from The SEED and Food Access deliveries.

WE Breastfeed & Chestfeed program supports parents feeding human milk in any way, in any amount. Parents can receive support in two ways. One-to-one telephone support with trained and experienced parent-volunteers or weekly drop-in Breastfeeding & Chestfeed Cafes. Cafe is offered in an online format and is a great place for parents to socialize with other breastfeeding / chestfeeding families and get support with challenges. 
 
Become trained to offer support and lead group sessions providing feeding & parenting support to other parents. Keep logs of supporting, track hours, remain in good communication with the program coordinator. 
 
 
IMPORTANT Information:  
 
Volunteers in this role are required to have had breastfeed or chestfeeding experience. Please consider joining this program as an administrative support or in a health promotion role if you do not have breast or chest feeding experience.  
 
Guelph CHC acknowledges the importance of Peer-led work and offers 20 hours of training for this position but also ask for a minimum 6 month commitment. 
 
One of our strategic aims to build capacity in our community. Volunteers in this role must have a connection to Guelph.
