DutiesDescription: Screen patients for COVID, confirm appointment time & identification on person 
Location: Outside Vestibule 
Required PPE: Face mask, vest, and face shield, provided by clinic 
Required Materials: COVID Question Document, provided by clinic 
Roles and Responsibilities: 
• Greets patient when they enter line up and ensure everyone is spaced appropriately 
• Asks patient to confirm appointment time 
• Asks patient if they have on their person their health card or alternative ID (accepted IDs listed in COVax, a software created and provided by the provincial government for COVID-19 vaccine administration) 
• Ask the 3 COVID questions on document 
• Direct patients into vestibule when empty 
• No food or drink is allowed in lines

Volunteers are not covered by WSIB and the communication of risk, training and ongoing support is the responsibility of the Linamar (Skyjack) Vaccination Clinic.
