Description (Service) | Currently no in-person groups at this site due to COVID-19. Virtual groups offered to program clients. Community outreach program that assists people with aphasia to engage and participate in conversation using supportive communication techniques * group sessions facilitated by speech-language pathologist, communicative disorders assistant and social worker * education and support for caregivers **Part of the Integrated Care Pathway for Stroke, in cooperation with Adult Day Programs. **Does not offer individual speech-language therapy and is not available to residents in long term care facilities |

Eligibility | People with aphasia resulting from stroke or brain injury * people who have completed or nearly completed their inpatient or outpatient course of therapy with a speech-language pathologist * people with aphasia that is likely to remain the same or improve * people who are able to participate in groups and maintain attention for longer periods of time |
