Description & Services
Description (Service) Community outreach program that assists people with aphasia to engage and participate in conversation using supportive communication techniques * group sessions facilitated by speech-language pathologist, communicative disorders assistant and social worker * education and support for caregivers 
 
**Part of the Integrated Care Pathway for Stroke, in cooperation with Adult Day Programs. 
 
**Does not offer individual speech-language therapy and is not available to residents in long term care facilities
Hours Hours vary * conversation groups are scheduled once a week - see website for schedule of times and locations
Areas Served Guelph ; Wellington County
Eligibility People with aphasia resulting from stroke or brain injury 
* people who have completed or nearly completed their inpatient or outpatient course of therapy with a speech-language pathologist 
* people with aphasia that is likely to remain the same or improve 
* people who are able to participate in groups and maintain attention for longer periods of time
Application Referrals accepted by phone at 519-824-6000 ext 4357, or fax at 519-767-3442 by completing the Waterloo Wellington Regional Aphasia Program Referral Sheet available on the website * self referrals and referrals from community partners (Waterloo Wellington Local Health Integration Network, hospitals, Community Support Services (CSS) agencies)
