Description (Service) | COVID-19 Update: Program temporarily closed; contact for program status and referral information. Community outreach program for adults who want to remain in their own homes or with families and are coping with effects of moderate to severe Alzheimer's disease or other forms of dementia including Young Onset Dementia. Services include: * assessment * individual program planning * supervised therapeutic social / recreational activities * structured routine environment * behaviour management * care coordination * support, counselling and advocacy for families **Transportation may be available or support coordinating with community transportation service if family is unavailable** |
