| Description (Service) | Coordinates patient care in order to assess and treat injuries or illness * team includes Family Doctors, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Registered Kinesiologists, Registered Pharmacists and Mental Health Counsellors Patients may attend group sessions which address common health issues and treatment programs. Workshops and programs include * Stop Smoking * INR Clinic * Primary Care At Home Program * Diabetes Education * Foot Care Clinic * 24-hour BP Monitoring * Wellness Matters workshops/webinars * Mental Health workshops * Chronic Pain Management Program * Postpartum Mood Disorder Support Group * Raising Competent Eaters workshops * YMCA walking group Family Health Team works with 95 Guelph Family Doctors to give people access to a number of primary health care services from a variety of team members. |
