Description (Agency) Team of health professionals providing a full range of locally based, publicly funded primary health care services.
Description (Service) Access to a number of primary health care services provided by a family doctor and various health team members. Services provided by: 
* nurse practitioners / specialists 
* physician assistant 
* mental health therapist 
* social worker 
* occupational therapist 
* clinical telemedicine nurses 
* dietitians 
* pharmacists 
* health promoter 
NOTE: patients see their family doctor during regular office hours
Hours Office hours 8:30 am–5 pm * some evening programs available
Areas Served Centre Wellington - including Elora, Fergus and surrounding area ; Wellington North - including Arthur and surrounding area
Eligibility Majority of services require referrals from a collaborating family physician of the Upper Grand Family Health Organization. To see one of the team members, make an appointment with your family physician to be referred, or call the Family Health Team for more information.
Application No direct enrollment. Referral from FHT family physician is required. See website for a list of FHT physicians.
