Description (Agency) Team of health professionals providing a full range of locally based, publicly funded primary health care services.
Description (Service) COVID-19 Update: Beginning Mar 16, 2020 Upper Grand FHT staff working out of the main office location at 143 Metcalfe Street in Elora are postponing home visits and providing appointments via telephone only. This includes Geriatric, Palliative and Chronic Disease Management Nurse Specialists, Geriatric and Palliative Social Workers, Mental Health Therapists, Occupational Therapist, Addictions Counselors, Pharmacist, Health Promoter and Dietitians. Access to these services will continue to be available to patients who are referred by their family physician. Group programs and clinics requiring pre-registration have also been cancelled until further notice. Appointments requiring in-person sessions will be rescheduled once current COVID-19 precautionary measures are no longer in place.

Nurse Practitioners and Physician Assistant are continuing to provide services in affiliated physician clinics across Centre Wellington until further notice.

Access to a number of primary health care services provided by a family doctor and various health team members. Services provided by:
* nurse practitioners / specialists
* physician assistant
* mental health therapist
* social worker
* occupational therapist
* clinical telemedicine nurses
* dietitians
* pharmacists
* health promoter/kinesiologist

NOTE: patients see their family doctor during regular office hours.

Hours Office hours 8:30 am–5 pm * some evening programs available
Areas Served Centre Wellington - including Elora, Fergus and surrounding area ; Wellington North - including Arthur and surrounding area
Eligibility Majority of services require referrals from a collaborating family physician of the Upper Grand Family Health Organization. To see one of the team members, make an appointment with your family physician to be referred, or call the Family Health Team for more information.
Application No direct enrollment. Referral from FHT family physician is required. See website for a list of FHT physicians.
