Description & Services
Description (Service) Residents of Ontario eligible for Ontario Health Insurance Plan (OHIP) coverage require an Ontario health card to access coverage for health care services paid for by the Ontario Health Insurance program.

Services partially or fully funded by OHIP include medically necessary physician services, hospital visits and stays, eligible dental surgery in hospital, eligible eye health services, podiatry, and certain practitioner services that are therapeutically necessary.

Eligibility Must have proof of Canadian citizenship or OHIP-eligible immigration status; proof of residency in Ontario and support of identity (document that displays both name and signature)
Application Individuals who have never had an Ontario health card must apply through ServiceOntario to obtain a photo health card:
* to complete a registration form for Ontario health insurance coverage, visit Apply for OHIP and get a health card
* provide three original documents (proof of citizenship, proof of residency, support of identity)
* visit the nearest ServiceOntario centre in person
