|Description (Service)||Interim mental health and addiction support for individuals transitioning into the community after inpatient psychiatric care or following a recent mental health crisis.
Services may include * phone support * individual multidisciplinary team assessment * consultations and contacts * group education and individual support sessions * support for medication management * peer supports
Service model supports persons with complex mental health and addiction concerns and integrates medical and recovery approaches to support individual, personal wellness goals.

To qualify for the Transitional Care Program, patients must:
* be in psychiatric crisis and require urgent and intensive services
* have an impaired ability to function in the community without support
* have sufficient judgment to maintain function in the community environment
* agree to participate in the program
* reside within the City of Guelph or Wellington County
* be between 18-65 years of age
|Application||Homewood in-patient referrals to Transitional Care are made through the in-patient clinical teams of Homewood Regional Services.
Community Access via 24-Hour Crisis Supports: Emergency departments at Guelph and Wellington local hospitals, and contact with the mental health crisis nurse.
Individuals who are in crisis should contact their doctor or go to the local community emergency department. If necessary, a consultation or assessment will be completed by the mental health crisis nurse at the emergency department.
