Description & Services
Description (Service) Outpatient treatment, support and rehabilitation for people with severe and persistent mental illness. Services include: 
* medication monitoring and supervision 
* crisis assessment and intervention 
* symptom assessment, management and education  
* supportive counselling 
* collaboration with families and agencies 
* life skills teaching 
* housing assistance 
* recreational support 
* vocational support 
* continuity of care 
* advocacy  
 
Multidisciplinary treatment team includes psychiatrist, nurses, social workers, occupational therapists, recreation therapists, addictions workers, program coordinator and program assistant. 
 
Referrals can be arranged directly or through Here 24/7 service.
