Description & Services
Description (Agency) Health centre that focuses on family and community health. Programs and services are delivered with a focus on individuals who experience unstable housing or homelessness, low income, recent immigration and language barriers, moderate to severe mental health challenges / addictions, moderate to severe disabilities, limited or no access to primary care, at-risk circumstances and social exclusion.
Description (Service) Health professionals assess and treat non-life-threatening injuries or illnesses and make referrals to other local health services and personal development groups. Services include clinical services, children's programs, childbirth preparation, parent and family support, multicultural outreach services, and ID clinic.

Community-led health teams develop programs to improve social supports and conditions that affect long-term health

Breastfeeding support - counselling and peer support for breastfeeding mothers

Chronic disease self-management - sessions to learn ongoing self-care (information about coping, health guidelines, medication usage, working with health professionals and informed treatment decisions)

Clinical dietitian - nutritional information and education; one-on-one, couple, family or group sessions for people with food and health-related concerns

Fitness and recreation - exercise to strengthen muscles, heart, lungs and healthy living programs

Foot care clinic - assesses and treats common foot problems, including basic foot care for diabetic and non-diabetic clients

Health outreach for people who are homeless - supports for people who are homeless and at risk of homelessness

Warm Connections Program Wed 10 am-12 noon, Downtown location, Lower Level, Atrium - coffee or tea and light snack * access to hot shower * opportunity to visit ID Clinic * social time * access to winter emergency kits (while supplies last)
