Description (Agency) Post-recovery support, education and programs for stroke survivors, their families and caregivers.
Description (Service) Information, resources and a network of mutual support through work with local peer support groups * local meetings, activities and seminars provide opportunities for stroke survivors to develop new skills, make new friends and gain a better understanding of what to expect

* Guelph and Area Peer Support meetings, held on the third Tue of the month 11:45 am-2:30 pm 
* Survive and Thrive Caregiver Group meetings, held on the second Thu of the month 12 noon-2 pm

Eligibility Stroke survivors, caregivers, health care professionals, family members, friends and any individuals interested in learning more about stroke - all ages welcome
