Description (Service) (Formerly Community Care Access Centre) 
 
Works to improve the quality and availability of local health care and connects people with care and support services at home and in the community, including: 
* hospital care 
* long-term care (help with admissions) 
* mental health and addictions services 
* home and community care 
* community support services 
* primary care (family doctor or nurse practitioner) 
 
Home and community care staff develop individualized care plans and coordinate services such as nursing, physiotherapy, occupational therapy and personal support for clients during recovery from illness or injury, and for people living with chronic illness or disability. 
 
**Referral to other local health and support services, which may include transportation, meal programs or day programs; (there may be a nominal fee for these support services)

Eligibility Anyone with a valid Ontario health card may be referred. Must meet eligibility criteria to access publicly-funded health services
