Description (Service) | (Formerly Community Care Access Centre) Programs and services include: * hospital care * long-term care (help with admissions) * mental health and addictions services * home and community care * community support services * primary care (family doctor or nurse practitioner) Home and community care staff develop individualized care plans and coordinate services such as nursing, physiotherapy, occupational therapy and personal support for clients during recovery from illness or injury, and for people living with chronic illness or disability. **Referral to other local health and support services, which may include transportation, meal programs or day programs; (there may be a nominal fee for these support services) |
