Description & Services
Description (Service) Residents of Ontario eligible for Ontario Health Insurance Plan (OHIP) coverage require an Ontario health card to access coverage for health care services paid for by the Ontario Health Insurance program * OHIP funds medically necessary physician services, dental services required to be performed in hospital and certain practitioner services that are therapeutically necessary. 
ServiceOntario delivers health card registration services (OHIP) on behalf of the Ministry of Health and Long-Term Care.

Eligibility Must have proof of Canadian citizenship or OHIP-eligible immigration status; proof of residency in Ontario and support of identity (document that displays both name and signature)
Application Individuals who have never had an Ontario health card must apply through ServiceOntario to obtain a photo health card.  
* complete a registration for Ontario health insurance coverage form (available on ServiceOntario website) 
* provide three original documents (proof of citizenship, proof of residency, support of identity) 
* visit the nearest ServiceOntario centre in person
