DescriptionHealth centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups 
 
Community-led health teams develop programs to improve the social supports and conditions that affect long-term health 
 
Services include clinical services, children's programs, childbirth preparation, parent and family support, Ontario Early Years Centre, multicultural outreach services, ID clinic 
 
Breastfeeding Support - Counselling and peer support for breastfeeding mothers  
 
Chronic Disease Self-Management - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions 
 
Clinical Dietitian - Nutritional information and education * one-on-one, couple, family or group sessions for people with food and health-related concerns 
 
Fitness and Recreation - Exercise to strengthen muscles, the heart, and the lungs * healthy living programs 
 
Foot Care Clinic - assesses and treats common foot problems * includes basic foot care for both regular and diabetic clients 
 
Health Outreach For People Who Are Homeless - Supports for people who are homeless, and those at risk of homelessness
